Do you have
or have you contracted with a "troubled program"?
This
questionnaire will help you determine if you are or have
contracted with a "troubled program" for teens. If you
answer yes to two or more of the following questions, you (or
with whom you have contracted) is in immediate need of
intervention. The intervention we recommend is to cease
operations of such facility as currently operated and/or remove
your child and/or all children from said facility. This
may be a matter of life and death as many children and families
have suffered debilitating and life-threatening results by
entrusting their well-being to fraudulent, deceptively-marketed,
and/or abusive facilities. |

operate without being properly licensed to provide services
offered?
(Special Qualifier: Many programs in
Utah are
licensed per se, but, are not licensed to provide the types of
services offered. Some are licensed only as private
schools or daycares and are not licensed as residential
facilities or mental health service providers.) |

have a history of abuse or repeated and substantiated reports of
abuse? (Special
Qualifier: If you are uncertain what constitutes "abuse", please

Does your troubled program
use ambiguous terms and refuse to answer simple and direct
questions? |

Does your troubled program
avoid communicating honestly and cover up or excuse complaints
and reports of abuse? |

model itself after or associate heavily with other fraudulent
and/or abusive programs? |

Does your troubled program
hold children 14-years old or older against their will without
court order and in violation of their due process rights? |

Does your troubled program
use Positive
Control Systems®, used by WWASPS, Aspen Education Group, and
many others? |

Does your troubled program
blame you/the parents for any failures of your/their child in
the program as opposed to holding themselves accountable for
their failures to provide proven and effective assistance to
families in crisis? |

Does your troubled program
include in contracts unconscionable waivers that remove any and
all liability for intentional and negligent harm befalling a
child in the program and place all liability with the families
who have entrusted the care of their child to the program?

match the warning signs at
www.heal-online.org/warn.htm and/or are they unable to
effectively and directly answer questions about their practices? |

Has your troubled program
changed ownership/names/etc. repeatedly to distance itself from
past wrongs while continuing in the same practices and/or with
the same staff that led to those
wrongs/lawsuits/complaints/etc.? |

Again, if you answered
yes to two or more of the above questions, you are dealing with a
troubled program and immediate intervention is highly recommended.
We urge you to stop operations immediately unless and until you are
effectively regulated and the care of children in your facility is
guaranteed as opposed to waived away by unconscionable contract
terms or other forms of coercion and intimidation.
If you have contracted with
a troubled program and are now unsure how to proceed, please go to the
program immediately and remove your child. Your child will only be
harmed by associating with a troubled program while living under their
deception and immoral control. For the sake and safety of your
