| HEAL received an e-mail on July 14th, 2011 from an individual stating he/she had been abused at this program. | ||||||||||||||||||||||||||||||||||||||||||||||||||||||||||||||||
| HEAL is in the process of reviewing the release form for this program. It is the only significant documentation we have for a review at this time. | ||||||||||||||||||||||||||||||||||||||||||||||||||||||||||||||||
The Cleveland Home Inc. (aka The Cleveland Home, Life Adventure Center of the
Bluegrass is licensed as a non-profit and Kentucky Corporation. It has
been licensed in the past as an orphanage.
HEAL was unable to find any evidence that The Cleveland Home/Life Adventure Center is a licensed child care facility. It does not appear to be properly licensed as a group home, residential services provider, or anything of that nature. We searched multiple license verification systems and directories and found no evidence that this program is properly licensed.
